Orthopaedic Doctor or Physiotherapist: Who Should You See First?
Medically reviewed by Dr. Mohsin Merchant, Orthopaedic Surgeon | Pain Cure Ortho & Rehab Clinic, Mazgaon, Mumbai |
You have had back pain for two weeks, or your knee hurts every time you climb the stairs. A friend says, "Go to a physio." Someone else says, "See a bone doctor first." Both sound reasonable, so who should you actually see?
The answer depends on your symptoms. In this article, we explain what an orthopaedic doctor does, what a physiotherapist does, and how to decide where to start. We also explain why many patients do best when both work together.
What does an orthopaedic doctor do?
An orthopaedic doctor (orthopaedic surgeon) is a medical doctor who specialises in bones, joints, ligaments, tendons, muscles and the spine. An orthopaedic doctor can:
- Examine you and find the cause of your pain
- Order and read tests such as X-rays, MRI and blood tests
- Diagnose conditions like slip disc, arthritis, ligament tears and fractures
- Prescribe medicines and give injections when needed
- Perform surgery, such as joint replacement or fracture fixation, when it is truly needed
- Plan your overall treatment, including physiotherapy
In short, the orthopaedic doctor answers the question: "What exactly is wrong, and what is the right treatment?"
What does a physiotherapist do?
A physiotherapist is a trained health professional who uses exercise, movement and physical therapies to reduce pain and restore function. A physiotherapist can:
- Assess your movement, strength, flexibility and posture
- Use therapies such as IFT, TENS, ultrasound and heat therapy to ease pain
- Teach exercises to strengthen weak muscles and improve joint movement
- Guide your recovery after surgery or injury
- Teach you how to sit, lift and move safely to prevent the pain from returning
The physiotherapist answers the question: "How do we get you moving well again, and keep you that way?"
When should you see an orthopaedic doctor first?
See an orthopaedic doctor first if you have any of these:
- Pain after a fall, accident or sports injury, especially with swelling or bruising
- A joint that looks deformed, locks, or gives way
- Pain that travels down the arm or leg, with numbness, tingling or weakness
- Pain that wakes you up at night or is present even at rest
- Pain that has lasted more than 2 to 3 weeks without improvement
- Swelling, redness or warmth in a joint, or fever along with joint pain
- Known conditions like osteoporosis, or a history of cancer, with new bone pain
Seek urgent medical care if back pain comes with loss of bladder or bowel control, numbness around the private parts, or sudden weakness in both legs. If an elderly person falls and cannot stand or put weight on the leg, they may have a hip fracture and need urgent care.
When can you start with a physiotherapist?
You can often start with physiotherapy for:
- Mild muscle strain or stiffness after exercise or long sitting
- Posture-related neck and back pain without nerve symptoms
- Recovery exercises after a problem has already been diagnosed by a doctor
- Rehab after surgery, as advised by your surgeon
If your pain does not improve within a few sessions, or new symptoms appear, a good physiotherapist will refer you to an orthopaedic doctor.
Why the best results come from both working together
Pain is not always what it seems. Knee pain may come from the hip. Shoulder pain may come from the neck. Back pain may be a simple strain, or it may be a disc pressing on a nerve. When an orthopaedic doctor diagnoses the problem first, physiotherapy can target the real cause instead of only the symptoms.
At the same time, many orthopaedic problems do not need surgery. The right exercises and therapy, planned around an accurate diagnosis, often relieve pain and restore movement. This is why at Pain Cure Ortho & Rehab Clinic in Mazgaon, the orthopaedic consultation and Physiotherapy are in the same clinic. Your doctor and physiotherapist follow the same plan, and your progress is reviewed by the same team.

Frequently asked questions
Can a physiotherapist diagnose my problem?
A physiotherapist can assess how you move and identify likely muscle or joint problems. However, conditions that need scans, medicines or surgery are diagnosed and managed by a doctor.
Will the orthopaedic doctor always suggest surgery?
No. Most orthopaedic problems, including most back pain, neck pain and early arthritis, are treated without surgery. Surgery is considered only when other treatment is not enough.
How many physiotherapy sessions will I need?
It depends on your condition and how long you have had it. Your physiotherapist will share an estimate after the first assessment.
Can I see both on the same visit?
At our clinic, yes. After your orthopaedic consultation, physiotherapy can be started in-house as part of the same plan.
Not sure where to start? Start with one visit.
At Pain Cure Ortho & Rehab Clinic, Dr. Mohsin Merchant, Orthopaedic Surgeon, assesses your problem and, when suitable, starts your physiotherapy in the same clinic.
