Knee Pain After 40: Causes, Warning Signs and What Really Helps

Medically reviewed by Dr. Mohsin Merchant, Orthopaedic Surgeon | Pain Cure Ortho & Rehab Clinic, Mazgaon, Mumbai |

Climbing stairs feels harder than it used to. Getting up from the floor takes a little planning. Your knees feel stiff when you get out of bed. If you are over 40, these changes are very common. The good news is that knee pain after 40 can usually be managed well, and in most people it does not need surgery.

Here is what causes it, which signs mean you should see a doctor, and what actually helps.

Why does knee pain become common after 40?

1. Wear of the knee cartilage (osteoarthritis)

The ends of the knee bones are covered with smooth cartilage. Over the years, this cartilage can slowly thin and wear. This is called osteoarthritis. It often affects the inner side of the knee first and causes pain while walking, stiffness after rest, and sometimes swelling. Read more on our Knee Arthritis page.

2. Weaker thigh and hip muscles

After 40, muscle strength naturally reduces unless we work to maintain it. The thigh muscles (quadriceps) act like shock absorbers for the knee. When they are weak, more load goes directly through the joint.

3. Body weight

When you walk, your knees carry several times your body weight with each step. Even a small amount of extra weight adds up over thousands of steps a day.

4. Old injuries

A ligament or meniscus injury from years ago, even if it seemed to heal, can make the knee more likely to wear early.

5. Daily habits

Sitting cross-legged or squatting for long periods, using Indian-style toilets, and climbing many stairs every day all put extra load on the knee.

Warning signs: when to see an orthopaedic doctor

  • Knee pain that lasts more than 2 to 3 weeks or keeps coming back
  • Swelling, warmth or redness around the knee
  • The knee locks, catches or gives way while walking
  • You cannot fully straighten or bend the knee
  • Pain at night or at rest
  • Your legs are starting to look bowed
  • Pain is stopping you from walking, working or sleeping well

Seek urgent care if the knee is suddenly very swollen, red and hot, especially with fever.

What really helps knee pain after 40

Strengthening exercises

Strong muscles are one of the best treatments for knee pain. Simple exercises such as straight leg raises, mini squats with support, and step-ups, done correctly and regularly, can reduce pain and improve walking. A physiotherapist can teach you the right exercises for your knee and progress them safely. See our Physiotherapy page.

Staying active, the right way

Rest alone often makes stiffness worse. Walking on flat ground, cycling on a stationary bike and swimming are usually kind to the knees. Avoid activities that cause sharp pain.

Managing weight

Losing even a few kilograms can noticeably reduce the load on your knees.

Small changes at home

  • Use a chair instead of sitting on the floor when possible
  • Use a western-style toilet or a raised seat
  • Hold the railing when climbing stairs, and lead with the stronger leg going up
  • Wear comfortable shoes with good cushioning

Non-surgical treatments at the clinic

At Pain Cure Ortho & Rehab Clinic, knee treatment usually starts with non-surgical options such as physiotherapy, ozone/ERP therapy and IR lamp therapy, chosen after an assessment of your knee. See Knee Treatment.

What if non-surgical treatment is not enough?

When arthritis is advanced and pain affects daily life despite proper treatment, knee replacement may be advised. Options include partial (unicondylar) knee replacement, when only one part of the knee is worn, and total knee replacement.

Dr. Mohsin Merchant performs Robotic Knee & Hip Replacement using the Mako robotic system at Wockhardt Hospital, Mumbai Central. The surgery is planned from a CT-based 3D model of your knee, and the robotic arm helps the surgeon follow that plan precisely. Rehab after surgery is done at our Mazgaon clinic.

Most people with knee pain after 40 never need surgery. But when it is needed, modern techniques and good rehab help patients get back to walking with confidence.

Myths about knee pain after 40

"Knee pain is just age. Nothing can be done."

Not true. Exercise, physiotherapy and the right treatment help most people significantly.

"If my knee hurts, I should stop walking."

Usually not. Gentle, regular walking helps the joint. Ask your doctor what is right for you.

"Knee replacement is the only real solution."

Surgery is for a small number of patients with advanced arthritis. Most people improve with non-surgical care.
